Search Instructions

Enter the facility name and select the Search button to see a list of matching health care facilities.

You may also add the street name, city / town, or county area to further limit the search results. To see a list of all facilties, submit a blank search.

Use the wildcard "%" to customize your search. For example, a search for "%Cafe" might return "Martha's Cafe", "Java Cafe", and "Cafe Marina" because they all have the word "Cafe". To see a listing of all facilities that start with "A" search for "A%".

Data from July 1, 2002 to currently posted surveys are presented in the search. Inspection reports are posted on a weekly basis.
