A health inspection report is a "snapshot" of the day and time of the inspection. On any given day, an establishment could have more or fewer violations than noted here. Inspections are based on regulations to eliminate risk factors for illness or injury. Every violation is coded indicating the degree of risk in causing an illness or injury.

  • Critical, Major, or Foodborne Illness Risk Factor Violations: These violations are more likely than other violations to contribute to illness or injury. Inspectors work with operators to make sure that these violations are corrected whenever feasible before they leave the establishment. A re-inspection may be scheduled to verify compliance.
  • Non-Critical, Minor or Good Retail Practice Violations: These violations are not directly related to the cause of illness or injury, but if uncorrected, could negatively affect the operation of the facility, such as maintenance and cleanliness.

Inspector Comments: The temperature of water at the Toddler 1 classroom combined faucet toddler hand sink (70°F) did not range between 90-120°F. The length of time that the metered faucet hand sinks remained on without reactivating the faucet was less than 10 seconds at the following locations: School Age 2 classroom main hand sink (7 seconds) and boys' bathroom hand sink (1 second). Corrective action: The hand sink water temperature was corrected to measure between 90-120°F, and the metered faucets were modified to remain on for at least 10 seconds.

Inspector Comments: The mechanical air drying device at the School Age 1 classroom hand sink was not working, and paper towels were not provided. Corrective action: Paper towels were provided at this sink.

Inspector Comments: A child in the Pre-K classroom did not wash his hands after using the toilet. Corrective action: The inspector informed the teacher, who had the child wash his hands properly.

Inspector Comments: The following foods were stored greater than 41°F: (in the Infant 1 room) milk (53°F), (in the left Kitchen refrigerator) milk (46-50°F), and rice milk (50°F). Shepherd's pie (113-120°F) from a delivery was received at a temperature below 135°F. Corrective action: The Infant 1 room refrigerator thermostat was modified to hold at or below 41°F, and receiving temperatures were discussed with the person in charge. Follow-up required for left kitchen refrigerator.

Inspector Comments: A thermometer was not provided in the left kitchen refrigerator. The kitchen dish washing area hand sink did not have a sign that told employees to wash their hands.

Inspector Comments: The metal bars above the slides of the outdoors play castle were soiled with rust.

Inspector Comments: Holes in the wall were located underneath the upper play area in Pre-K 1. A hole in the wall was located at the base of the stairs in the Kindergarten classroom. Multiple air fresheners were located in Intermediates 2. The blue sofa in the pre-primary classroom was torn and in disrepair. Paint on the ventilation of the air conditioning system underneath the upper play area in the Twos classroom was chipped. The floor level temperature (66°F) in the Infant 1 room was less than 68°F. A spray can of air and fabric odor eliminator was stored under the Toddler 2 room diaper changing hand sink. Peppermint and almond essential oils being used as part of educational activities in the Infant 1 room were observed in the cabinet.

The temperature of hot water at the Toddler 1 room children’s hand sink (85°F) and at the adult restroom hand sink in the infant/toddler building (87°F) did not range between 90-120°F.
