Inspector Comments: 3-101.11 Safe, Unadulterated and Honestly Presented (P) -Several expired containers of infant formula were stored on the shelf with wholesome containers in the baby section of the facility. Corrective Action: Staff removed the expired formula containers from the shelf and education was provided regarding checking for product quality/safety.

Inspector Comments: 4-501.11 Good Repair and Proper Adjustment-Equipment (C) -The gaskets on all of the customer self service glass doors to the walk in refrigerator were torn.

Inspector Comments: 5-202.13 Backflow Prevention, Air Gap (P) -The mop sink located in the back loading dock/storage area did not have an atmospheric vacuum breaker and a hose was present that reached the basin of the sink and did not provide an air gap greater than 1 inch. Corrective Action: Staff will shorten the hose so that an air gap of at least 1 inch is present between the end of the hose and the flood rim.

Inspector Comments: 5-501.115 Maintaining Refuse Areas and Enclosures(C) -The outdoor dumpster enclosure located outside the loading dock area was heavily soiled with trash and debris.
