A health inspection report is a "snapshot" of the day and time of the inspection. On any given day, an establishment could have more or fewer violations than noted here. Inspections are based on regulations to eliminate risk factors for illness or injury. Every violation is coded indicating the degree of risk in causing an illness or injury.

  • Critical, Major, or Foodborne Illness Risk Factor Violations: These violations are more likely than other violations to contribute to illness or injury. Inspectors work with operators to make sure that these violations are corrected whenever feasible before they leave the establishment. A re-inspection may be scheduled to verify compliance.
  • Non-Critical, Minor or Good Retail Practice Violations: These violations are not directly related to the cause of illness or injury, but if uncorrected, could negatively affect the operation of the facility, such as maintenance and cleanliness.

Inspector Comments: A shutoff valve was located downstream of the atmospheric vacuum breaker on the mop sink. The shutoff valve can be removed or a hose bib needs to be installed.

Inspector Comments: The sanitizer solution in the infant room was less than 150 ppm quaternary ammonium residual. Corrective Action: The bottle was refilled to reflect the correct concentration.

Inspector Comments: Per staff, bottle warmers are not sanitized after being washed and rinsed. Corrective Action: Bottle warmers are required to be washed, rinsed, and sanitized daily.

Inspector Comments: Test strips were not available at the facility for measuring sanitizer and disinfectant concentrations. Corrective Action - Staff will obtain test strips.

Inspector Comments: Staff was not allowing a 10 minute contact time for disinfectants per manufacturers labeling in all rooms. Corrective Action - Education was provided regarding following manufacturers labeling for contact time of disinfectants.

Inspector Comments: The sanitizer solution in the three compartment sink measured below 200 ppm quaternary ammonia residual. Corrective Action - Staff remade the sanitizer with the correct concentration.

Inspector Comments: Radon test results were not readily available by the facility at the time of inspection. Corrective Action - Director will obtain radon test results and send to TCHD. Several chemical spray bottles were not labeled as to their contents in the Toddler A room. Corrective Action - Staff labeled all chemical spray bottles.

: Art and Science Materials
Inspector Comments: Food used for arts and sciences was not labeled "not for consumption". Corrective Action - Staff labeled all required items.
