Toxic substances properly identified, stored & used
Inspector Comments: The cook line sanitizer bucket located next to the hand washing sink measured greater than 50 parts per million chlorine residual. Both of the sanitizer buckets located in the front dining room measured greater than 50 parts per million chlorine residual. Corrective action: all of the above sanitizer buckets were remade and the concentration was confirmed to be 50 parts per million chlorine residual.

Insects, rodents, & animals not present
Inspector Comments: A mouse (dead) was observed next to the facilities water heater.
45 - Good Retail Practices
Single-use / single-service articles: properly stored & used
Inspector Comments: The re-use of single use peeled garlic containers were observed storing various spices throughout the facility.

Food & non-food contact surfaces cleanable, properly designed, constructed, & used
Inspector Comments: Multiple shelves located throughout the kitchen area were lined with aluminum foil and/or cardboard. Multiple torn door gaskets were observed at the three-door reach-in refrigerator located in the front service area. Food was observed being stored in plastic grocery store bags in the three-door freezer located in the front service area.

Inspector Comments: A sprayer head was connected to the hose attached to the facilities utility sink faucet thus placing the atmospheric vacuum breaker under continuous pressure.

Toilet facilities: properly constructed, supplied, & cleaned
Inspector Comments: The trash cans located in the women's restroom did not have lids.

Inspector Comments: The facilities exterior dumpster did not have a drain plug.

Adequate ventilation & lighting; designated areas used
Inspector Comments: Multiple light bulbs located beneath the cook line ventilation hood were non-functional. Multiple light shields located on the cook line were either cracked and chipped or missing.
