Inspector Comments: The temperature of the Teriyaki Chicken Salad in the front service cold holding unit was 52°F at the time of inspection. Corrective Action: Teriyaki Chicken salad was discarded during the time of inspection (see voluntary condemnation).

Inspector Comments: 3-501.19 Time as a Public Health Control (P) Almond milk (58°F) at the customer self-service station on a 6-hour time as a public health control plan was not marked with a discard time, and a written plan was not available. Corrective action: The remaining almond milk was discarded, and the facility was emailed a 6 hour time as a public health control plan template.

Inspector Comments: An employee was observed handling money and then proceeded to put on gloves without first washing their hands. An employee was observed handling dirty and then clean dishes without first washing their hands. An employee was observed wiping their hands on an apron and then proceeded with food preparation without first washing their hands. Discussed importance of proper handwashing with manager to review with employees.

Inspector Comments: The dish machine failed to produce a concentration of 50-200 ppm chlorine residual. Corrective Action: The air bubble in the supply line was removed allowing the machine to function properly.

Inspector Comments: Bread was stored directly under the drain line of the steam table unit.

Inspector Comments: The bottom vent cover of the middle salad reach-in cooler unit was soiled with dust.

Inspector Comments: An unapproved employee beverage was stored in the reach-in refrigerator at the main preparation line. Corrective Action: The beverage was promptly removed and proper storage location of employee beverages was discussed with management.

Inspector Comments: The front service milk reach-in refrigerator door gaskets were soiled with bread crumbs. Caulking to the ware wash area was soiled with mold.
