A health inspection report is a "snapshot" of the day and time of the inspection. On any given day, an establishment could have more or fewer violations than noted here. Inspections are based on regulations to eliminate risk factors for illness or injury. Every violation is coded indicating the degree of risk in causing an illness or injury.

  • Critical, Major, or Foodborne Illness Risk Factor Violations: These violations are more likely than other violations to contribute to illness or injury. Inspectors work with operators to make sure that these violations are corrected whenever feasible before they leave the establishment. A re-inspection may be scheduled to verify compliance.
  • Non-Critical, Minor or Good Retail Practice Violations: These violations are not directly related to the cause of illness or injury, but if uncorrected, could negatively affect the operation of the facility, such as maintenance and cleanliness.

Inspector Comments: Chipped and peeling wall/trim paint was observed in the Toddler and Transitional Preschool 1 rooms. The caulking at the hand sink in the media room was is disrepair.

Inspector Comments: Foam blocks in the Imagination Station were peeling and in disrepair.

Inspector Comments: Illness surveillance logs were not kept up to date. Corrective Action: Ensure illness logs are maintained and all fields are filled out to completion.

Inspector Comments: The facility did not have a sanitizer solution available. Follow-up needed.

: Diapering
Inspector Comments: An employee in the Transitional Preschool 1 room was observed immediately wiping the disinfectant off the diaper changing table without observing the contact time. Corrective Action: Education provided; Director will ensure all employees know the correct contact time for their disinfectant solution. An employee in the Infant room did not remove dirty gloves before putting on a clean diaper. An employee in the infant room did not rinse off the soap and water with water before applying disinfectant to the diaper changing table. Corrective Action: Education provided on correct diapering procedure.
