The concentration of quaternary ammonia was greater than 400 parts per million in all of the sanitizer spray bottles in the kitchen. A food container was re-used to store chemicals in the chemical storage area.

Unwholesome bell peppers and unwholesome oranges were present in boxes with wholesome food items in the walk-in refrigerator. Cooked noodles in sausage sauce was 51°F after improperly cooling overnight in the walk-in refrigerator. A can of tomato ketchup was a dented bottom seam was stored with wholesome cans on the can storage rack. Two chemical spray bottles were not labeled as to their contents under the front counter. Chemicals were stored in plastic water bottles in the chemical storage area. Lighter fluid was stored next to and above customer single service items in the dry storage area.
