The inspection history for the business you selected is displayed below. Each inspection may have zero or more violations. A health inspection report is a "snapshot" of the day and time of the inspection. On any given day, an establishment could have more or fewer violations than noted here. Inspections are based on regulations to eliminate risk factors for illness or injury. Every violation is coded indicating the degree of risk in causing an illness or injury.

  • Critical, Major, or Foodborne Illness Risk Factor Violations: These violations are more likely than other violations to contribute to illness or injury. Inspectors work with operators to make sure that these violations are corrected whenever feasible before they leave the establishment. A re-inspection may be scheduled to verify compliance.
  • Non-Critical, Minor or Good Retail Practice Violations: These violations are not directly related to the cause of illness or injury, but if uncorrected, could negatively affect the operation of the facility, such as maintenance and cleanliness.

Inspector Comments: Operational floor thermometers were not present in infant and toddler rooms. A piece of raw wood was observed behind the hand sink in the Toddler 2 room. An aerosolized air freshener was present in the staff restroom.

Inspector Comments: An employee in the Toddler 2 room stated that water bottles left on site are cleaned weekly. Corrective Action: Education provided; discussed performing a daily wash, rinse, and sanitize on bottles kept on site at the facility. Recommended sending water bottles home at the end of the day.

Inspector Comments: Paper towels were stored in baskets and not protected from contamination throughout the facility. Corrective Action: Discussed paper towel storage with Director. Recommended using covered containers or wall dispensers.

Inspector Comments: A crib sheet in the infant room was visibly soiled. Corrective Action: Discussed removing sheets from cribs when they become visibly soiled with body fluids.

Inspector Comments: Sanitizer solution was mixed with soap in the Preschool room. Corrective Action: The Director remixed the solution.

Inspector Comments: The disinfectant solutions throughout the facility were less than 2400 ppm chlorine residual. Disinfectant solution was not available in the Toddler 2 room. Corrective Action: The Director remixed the solutions to reflect the correct concentration. Disinfectant solution was provided to the Toddler 2 room.

Inspector Comments: Food debris was observed in the hand sink in the kitchen. Corrective Action: Discussed using the hand sink only for washing hands with the kitchen employee.

: Staff Clothing for Infant Care
Inspector Comments: Clothing worn to and from work by staff members caring for infants is not replaced or covered by a smock.

Inspector Comments: Multiple bottles were not labeled with the infant's name or a date in the infant room refrigerator. The facility did not have a process in place to differentiate breast milk from formula. Per employee, the bottle warmer is not washed, rinsed, and sanitized daily. Follow-up required.

Inspector Comments: The diapering trash bin was uncovered in the infant room. The diaper changing pad in the infant room was torn. An employee changing a diaper in the infant room did not remove their gloves before touching clean diapers and other items. Children in the infant and toddler 2 rooms were returned to play without having their hands washed after a diaper change. An employee in the infant room did not allow the bleach disinfectant to remain on the diaper changing pad for the required contact time. An employee in the toddler 2 room did not disinfect the diaper changing table after a diaper change. Follow-up required.
