The hand sink adjacent to the food preparation sink was not properly attached to the wall. Cardboard was observed lining the shelf above the cold top refrigerator adjacent to the cook line hand sink.

A sprayer head was attached to the end of a hose thus placing the atmospheric vacuum breaker under continuous pressure at the facilities mop sink.
