A health inspection report is a "snapshot" of the day and time of the inspection. On any given day, an establishment could have more or fewer violations than noted here. Inspections are based on regulations to eliminate risk factors for illness or injury. Every violation is coded indicating the degree of risk in causing an illness or injury.

  • Critical, Major, or Foodborne Illness Risk Factor Violations: These violations are more likely than other violations to contribute to illness or injury. Inspectors work with operators to make sure that these violations are corrected whenever feasible before they leave the establishment. A re-inspection may be scheduled to verify compliance.

A 1/4th inch gap was present under the door of the firefly and bumblebees room and was not maintained for pest entry. The door hinge on the door for the sleeping mats cabinet in the dragonfly room was in disrepair.

Inspector Comments: Multiple books in classrooms throughout the facility were torn and in disrepair. The pillows and a pink plush sleeping mat in the fireflies room were visibly soiled and not being laundered weekly per staff. The soft toys and pillows in the bumblebee room were visibly soiled. The ABC posters located on the wall in the bumblebee room were torn and in disrepair. The corners of the play carpets in the ladybug room were torn and in disrepair. The chair cover and the baby bouncer cover in the infant room were torn and in poor repair.

Inspector Comments: The ill child log did not include all necessary information, including the date/time returned. The facility was not routinely recording children illnesses from February 2019- October 2019. Corrective Action: TCHD left an illness surveillance form with the facility and discussed logging all required information and illnesses.

Inspector Comments: An employee in the ladybugs room stated that children's hands were not being washed when first arriving to the classroom in the morning. Corrective action: Education was provided regarding proper hand washing times.

Inspector Comments: Per staff, the hard toys were not being washed, rinsed, and sanitized at the correct frequency per the age group and soft toys were not being laundered weekly in several classrooms. Corrective action: Education was provided on implementing a wash, rinse, and sanitize procedure and correct time frames. The sanitizer being used for toys and tabletops throughout the facility measured above 200ppm chlorine residual. Corrective action: Education was provided on proper sanitizer concentrations. Per staff, water bottles were being dumped and filled in the hand sink adjacent to restrooms in the ladybugs classroom. Spoons were observed in the basin of the hand sink adjacent to the diaper changing station in the ladybug classroom and were going to be washed. Corrective Action: Education was provided regarding not using a sink not in conjunction with toileting and diapering activities for bottle filling/cleaning toys and equipment.

Inspector Comments: The hand washing sink in the kitchen was being used to fill sanitizer and disinfectant bottles. Sanitizer and disinfectant bottles were stored in the basin of the hand sink in the kitchen. Corrective action: Education was provided on using hand sinks for hand washing only. TCS and ready-to-eat foods being stored in the refrigerator for longer than 24 hours were not labeled with an opened or discard date. Corrective action: Education provided on date marking and guidance left on-site.

Inspector Comments: The can opener in the kitchen was rusty and soiled. Food debris was observed in the cabinets and drawers throughout the kitchen.

Inspector Comments: Spray bottles containing chemicals throughout the facility were not labeled with their contents. Corrective Action: Staff labeled each of the chemical spray bottles. Chemicals were stored next to and with children snacks in the butterflies room. Medication was being stored above and next to children's snacks and single service items in the ladybug room. Corrective Action: Staff removed the chemicals and medication and placed them in an approved location.

Inspector Comments: Sensory bottles were not labeled as to their contents in multiple rooms throughout the facility. Baby food jars being used as sensory containers in the caterpillar room were not labeled as "not for consumption" or with their contents. Several sensory bottles throughout the facility did not have their lids glued shut. Corrective Action: Education was provided on properly labeling sensory bottles with their contents and staff will glue the lids of the sensory bottles shut.
