Inspections are based on regulations to eliminate risk factors for illness or injury. Every violation is coded indicating the degree of risk in causing an illness or injury.

  • Critical, Major, or Foodborne Illness Risk Factor Violations: These violations are more likely than other violations to contribute to illness or injury. Inspectors work with operators to make sure that these violations are corrected whenever feasible before they leave the establishment. A re-inspection may be scheduled to verify compliance.

Inspector Comments: The teacher in the Beginners A classroom was observed not washing children's hands after eating before returning to playing with toys. A staff member in the Infant B classroom stated that children under 6 months of age have their hands washed with a wipe after diapering. Corrective Action: Education was provided regarding proper hand washing procedures and times.

Inspector Comments: The toys and tabletops sanitizer located in spray bottles throughout the classrooms and in the kitchen measured greater than 200 ppm chlorine residual. The sanitizer used in the mouthed toy bucket in the Infant C classroom measured greater than 400 ppm quaternary ammonia residual. Corrective Action: Staff added water to each of the sanitizer containers to dilute to the correct concentration and education was provided regarding mixing instructions. Per staff, the hard toys were not being washed, rinsed, and sanitized daily and soft toys were not being laundered weekly in the Beginners A classroom. Corrective action: Education was provided on implementing a wash, rinse, and sanitize procedure for hard toys and correct sanitizing time frames for hard and soft toys.

Inspector Comments: Staff members in multiple classrooms stated that they would use sanitizer instead of disinfectant on high hazardous bodily fluid clean up. Corrective Action: Education was provided with each teacher regarding using disinfectant when cleaning bodily fluids.

Inspector Comments: The thermometer located in the kitchen was out of calibration by more than 2°F. A chemical spray bottle located in the kitchen was unlabeled as to its contents. A chemical tablet bottle was stored in a drawer with clean utensils. An employee in the kitchen was observed washing their hands for less than 20 seconds prior to donning gloves and preparing food. Corrective Actions: Staff calibrated the thermometer in ice water. All chemicals were labeled and placed in approved locations away from clean utensils. Education was provided regarding proper hand washing procedures.

Inspector Comments: Children in the Pre K classroom were observed using a cup with no handle to scoop cereal into bowls from a bag during family style meal time. Corrective Action: Education was provided regarding providing a scoop with a handle to limit bare hand contact with ready to eat foods.

Inspector Comments: A spray bottle in the Infant A classroom was labeled as water but contained soapy water instead. Several bottles of chemicals were stored in a container with clean dishes in the Infant B classroom. Staff medication was being stored in a cabinet above children's snacks and and clean dishes in the Infant B classroom. Chemical spray bottles were stored hanging from wire racks above the food preparation areas/clean dishes area in the Infant A, Infant B, and Infant C classrooms. Corrective Action: Staff labeled the soapy water spray bottle and moved the chemicals and medication to approved locations to prevent possible contamination.

Inspector Comments: Sensory bottles in classrooms throughout the facility were not labeled as to their contents. A sensory bottle containing food and baking soda used for arts and crafts/science projects were not labeled as "not for consumption". Corrective action: Education was provided on appropriately labeling sensory bottles with their contents and food being used for arts and crafts, science projects, and sensory tables.

Inspector Comments: Two 6 pound 9 ounce cans of diced peaches were stored dented and with other cans on the can rack in the kitchen dry storage area. Corrective Action: Cans were removed from the rack and isolated for return to supplier and kitchen staff were educated on the regulation for dented cans. Cream cheese kept for longer than 24 hours in the kitchen refrigerator was not date marked. Corrective Action: Cream cheese was dated with date opened and date it should be discarded.
