A health inspection report is a "snapshot" of the day and time of the inspection. On any given day, an establishment could have more or fewer violations than noted here. Inspections are based on regulations to eliminate risk factors for illness or injury. Every violation is coded indicating the degree of risk in causing an illness or injury.

Inspector Comments: Packaging tape was observed on the outside surface of the Infant 3 room refrigerator making it not smooth and easily cleanable. The caulking on the side of the Toddler 1 diaper changing table was severely cracked. The side of the counter top in the Toddler 1 classroom was chipped and covered with duct tape making the surface not smooth and easily cleanable. A piece of concave floor coving was missing from the mop sink area. A hole was present in the wall by the door in the kitchen.

Inspector Comments: Soap and paper towels were not supplied to the diaper changing hand sink in the Infant 3 classroom. Paper towels were not supplied to the diaper changing hand sink in the Toddler 1 classroom. CORRECTIVE ACTION: Facility restocked soap and paper towels to all hand sinks.

Inspector Comments: A staff member located in the Two's classroom stated that a vomit incident was cleaned with the toys and table tops bleach sanitizer solution and not the disinfectant solution. CORRECTIVE ACTION: Inspector provided education on high hazard bodily fluid clean up using an approved disinfectant solution and had the facility re-disinfect the area.

Scented sprays were located throughout the building in classrooms and restrooms. Floor thermometers were not available for monitoring temperatures in the two year old room, toddler 1 room, toddler 2 room, or nursery 3 room.

A child in the Toddler 2 classroom was observed picking their nose and returning to touching toys without having their hands washed. Children in the Toddler 1 classroom were observed coming into their classroom after playing outside, from the playground, and returning to play without first washing their hands.

Inspector Comments: An employee in the Infant 2 classroom was observed handling a ready-to-eat graham cracker with their bare hands immediately prior to serving it to an infant. Chemical spray bottles of diapering bleach disinfectant, sanitizer, and soap and water were stored with their spouts facing clean cups on the Toddler 2 classroom shelf. A bottle of full strength bleach was stored directly next to a clean water dispenser on the kitchen storage shelf. Single service cups, on a storage shelf in the Before and After School-Age classroom, were stored directly next to soap and sunscreen.

A bottle of breast milk measured 51°F inside the refrigerator in the Infant 1 classroom. A bottle of formula measured 55°F inside the refrigerator in the Infant 1 classroom. A bottle of formula in the Infant 2 classroom measured 49°F.

Inspector Comments: A water rinse, after bleach disinfection, was not performed during the diapering procedure in the following classrooms: Infant 1; Infant 2; Toddler 1; Toddler 2; and Young Preschool. The diaper changing mat in the Infant 2 classroom was torn. The trash receptacle for soiled diapers, in the Toddler 2 classroom, was not provided with a cover.

Inspector Comments: A squeeze bottle of dish detergent in the Infant 1 classroom was not labeled as to its contents. The sanitizer bin, for mouthed toys in the Infant 1 classroom, sanitizer concentration measured above 200 ppm chlorine residual. A chemical spray bottle containing glass cleaner was stored directly next to facial tissue and paper towels inside the Toddler 1 classroom cabinet. Chemical spray bottles of sanitizer and soap and water were in reach of children in the Preschool classroom. A chemical spray bottle of soap and water was in reach of children in the Pre-Kindergarten classroom. Chemical spray bottles of bleach disinfectant, sanitizer, and soap and water were in reach of children in the Young Preschool classroom.
