A health inspection report is a "snapshot" of the day and time of the inspection. On any given day, an establishment could have more or fewer violations than noted here. Inspections are based on regulations to eliminate risk factors for illness or injury. Every violation is coded indicating the degree of risk in causing an illness or injury.

  • Critical, Major, or Foodborne Illness Risk Factor Violations: These violations are more likely than other violations to contribute to illness or injury. Inspectors work with operators to make sure that these violations are corrected whenever feasible before they leave the establishment. A re-inspection may be scheduled to verify compliance.
  • Non-Critical, Minor or Good Retail Practice Violations: These violations are not directly related to the cause of illness or injury, but if uncorrected, could negatively affect the operation of the facility, such as maintenance and cleanliness.

Inspector Comments: In room F2 and room F4, cans of air fresheners used to mask noxious odors were observed in cabinets in the above classrooms. In room F2, carpeted mats were placed beneath both hand washing sinks and the water fountain.

Inspector Comments: In room F3, a linen chair used to isolate ill children was not being cleaned and sanitized at a minimum of once per week according to the teacher of that particular classroom. Corrective action: discussed with person in charge that the linen chair needs to be cleaned and sanitized weekly or whenever the surface becomes visibly soiled.

Inspector Comments: In room F1, F2, F3, and F4, the sanitizer spray bottles were not properly labeled with their intended use. In room F3, when sanitizing the table tops the teacher did not allow the sanitizer to air dry for at least 1 minute. In addition, a disposable paper towel was used to wipe up the sanitizer solution before the 1 minute contact time was achieved. Corrective action: all chemical spray bottles were properly labeled. Discussed proper sanitizing contact times with the person in charge.

Inspector Comments: Two dented cans of mandarin oranges were stored with wholesome cans on the bottom shelf adjacent to the single door refrigerator located in the kitchen. An employee beverage that did not contain a lid or a straw was stored next to bottles of mustard on the kitchen food preparation table. Corrective action: the two dented cans were marked "Do Not Use" and placed in the office to either be discarded or returned for credit. The employee beverage was relocated to an approved location and education was provided on proper employee beverage containers.

Inspector Comments: The light bulb located in the single door reach in refrigerator was non-functional. The hot water located at the kitchens three compartment ware washing sink measured 104°F after continuous use.

Inspector Comments: Date marking requirements were not being followed on potentially hazardous foods that remain in the facility for longer than 24 hours. Corrective action: date marking guidance hand out was provided via e-mail.

Inspector Comments: A spray bottle of an unknown chemical was unlabeled as to its contents in a cabinet located in room F4. Corrective action: person in charge discarded the unknown chemical spray bottle.

Inspector Comments: Several holes were observed in the wall behind the F1 classroom restroom handsinks. The floor tiles in the kitchen were cracked and allowing water to pool. Several holes were observed in the wall of the F3 restroom. At least twenty (20) foot candles of light were not provided in the F3 restroom.
