Inspector Comments: 2-101.11 Assignment (Pf) Upon arrival, the person in charge refused entry to the Environmental Health Specialists after identification was provided for a routine inspection. Corrective action: Discussed the requirement for person in charge at the facility during all hours of operation to allow entry.

The floor was soiled below the hand sink in the drive through area. The wall to the left of the walk in freezer was soiled. The ceiling tile was stained in the hallway by the restrooms.
