A health inspection report is a "snapshot" of the day and time of the inspection. On any given day, an establishment could have more or fewer violations than noted here. Inspections are based on regulations to eliminate risk factors for illness or injury. Every violation is coded indicating the degree of risk in causing an illness or injury.

  • Critical, Major, or Foodborne Illness Risk Factor Violations: These violations are more likely than other violations to contribute to illness or injury. Inspectors work with operators to make sure that these violations are corrected whenever feasible before they leave the establishment. A re-inspection may be scheduled to verify compliance.
  • Non-Critical, Minor or Good Retail Practice Violations: These violations are not directly related to the cause of illness or injury, but if uncorrected, could negatively affect the operation of the facility, such as maintenance and cleanliness.

Inspector Comments: The sandbox was not equipped with a cover.

Inspector Comments: A scent enhanced product plugged into the wall was used in room C37. Carpet was present on the step stool in the restroom between rooms C33 and C35 and between rooms C40 and C42.

Inspector Comments: The facility did not have hot water (between 90-120°F) at any of the hand sinks in the facility.

Inspector Comments: The yellow cloth bean bag chair in room C32 was not able to be laundered and did not have a removable cover on it.

: Handwashing and Hygienic Practices
Inspector Comments: Children in room C32 did not wash their hands after eating snack and going to the floor for the next activity. Corrective Action: Discussed when handwashing needed to occur with the staff. The children went to the sinks to wash their hands.

Inspector Comments: Sanitizer solution in room C34 was labeled as disinfectant. Sanitizer solution in room C25 had a concentration greater than 200ppm chlorine. Splashless bleach was used in sanitizer solution in room C39. Sanitizer solution in room C39 had a concentration greater than 200ppm chlorine. Sanitizer solution in room C40 had a concentration less than 50ppm chlorine. The toys and table top sanitizer in room C40 and C42 were not labeled for its use. Corrective Action: All sanitizer bottles were properly labeled as toys and tabletops sanitzer solution. All sanitizer bottles were checked to make sure they were between 50-200ppm chlorine if they were not at this level originally.

Inspector Comments: All diaper changing bleach solutions had concentrations less than 200ppm chlorine. Corrective Action: The diaper changing bleach solutions were remade so they were above 200ppm chlorine.

Inspector Comments: The facility did not have a metal stem thermometer that reads 0-220°F to check temperature for string cheese and milk. The facility was not sanitizing snack containers after use.

Inspector Comments: Staff said the dress-up clothes in room C32 were not laundered at a minimum of once a week. Staff said the awning for reading chairs in room C39 was not sanitized. The cloth couch in room C37 was sanitized by spraying bleach solution.

Inspector Comments: A couch in room C40 was frayed on the sides, deeming it not smooth and easily cleanable. A toy bucket in room C42 was severely cracked and reconstructed with tape. A wooden play house in room C33 was constructed of raw wood.
