CODE REFERENCE:
(8-501.11) No employer shall employ a person as a food handler for more than thirty (30) days unless and until such person shall provide to such employer written certification by a health care provider that:
(1) The person is immune from hepatitis A; or
(2) The person has been vaccinated against hepatitis A, including a booster shot within six (6) to twelve (12)
months of the original vaccination; or
(3) The person has received the initial vaccination against hepatitis A; however, said person must then provide the employer a health care provider's certification of the required booster shot within one year of the date of the original vaccination.
CORRECTIVE ACTION: Provide proof of primary/booster doses.

Light bulbs in the ceiling of the kitchen are not shielded.
CODE REFERENCE:
(6-202.11) (A) Except as specified in ¶ (B) of this section, light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed FOOD; clean EQUIPMENT, UTENSILS, and LINENS; or unwrapped SINGLE-SERVICE and SINGLE-USE ARTICLES.
(B) Shielded, coated, or otherwise shatter-resistant bulbs need not be used in areas used only for storing FOOD in unopened packages, if:
(1) The integrity of the packages cannot be affected by broken glass falling onto them; and
(2) The packages are capable of being cleaned of debris from broken bulbs before the packages are opened.
(C) An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.
CORRECTIVE ACTION: Discussed options with person in charge.

(a) There is no cover over the compressor on the Green World reach-in cooler at the bar.
(b) The drink rail above the four-compartment sink at the bar is damaged.
CODE REFERENCE:
(4-501.11) (A) EQUIPMENT shall be maintained in a state of repair and condition that meets the requirements specified under Parts 4-1 and 4-2.
(B) EQUIPMENT components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
(C) Cutting or piercing parts of can openers shall be kept sharp to minimize the creation of metal fragments that can contaminate FOOD when the container is opened.
CORRECTIVE ACTION: Repair these items as required.
(c) The cutting board in the kitchen is heavily-scratched.
CODE REFERENCE:
(4-501.12) Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and SANITIZED, or discarded if they are not capable of being resurfaced.
CORRECTIVE ACTION: Resurface or deep clean the scratches to remove soil.

(a) There are no drain plugs for the kitchen three-compartment sink.
CODE REFERENCE:
(5-202.11)(A) A PLUMBING SYSTEM shall be designed, constructed, and installed according to LAW.
(B) A PLUMBING FIXTURE such as a HANDWASHING SINK, toilet, or urinal shall be EASILY CLEANABLE.
CORRECTIVE ACTION: Provide at least two (2) fitted drain plugs.
(b) The drain tubing for the bar ice bin is not sloped and is connected to a wastewater pipe along the back of the three -compartment sink.
CODE REFERENCE:
(5-202.13) An air gap between the water supply inlet and the flood level rim of the PLUMBING FIXTURE, EQUIPMENT, or nonFOOD EQUIPMENT shall be at least twice the diameter of the water supply inlet and may not be less than 25 mm (1 inch).
CORRECTIVE ACTION: Reset the drain tubing so that it is properly sloped and then create an air gap = or >1" to floor drain, pipe or alternative method discussed.

(a) There is no coved base installed along interior walls of bar, brick walls in the toilet rooms, mop sink utility room (end of room where sink installed), and along three-compartment sink in the kitchen.
CODE REFERENCE:
(6-201.13) (A) In FOOD ESTABLISHMENTS in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than 1 mm (one thirty-second inch). (B) The floors in FOOD ESTABLISHMENTS in which water flush cleaning methods are used shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and SEALED.
CORRECTIVE ACTION: Install as required.
(b) There is absorbent ceiling tile inside the mop basin/utility room. I told person in charge to install only in the area of the mop basin.
CODE REFERENCE:
(6-101.11) (A) Except as specified in ¶ (B) of this section, materials for indoor floor, wall, and ceiling surfaces under conditions of normal use shall be:
(1) SMOOTH, durable, and EASILY CLEANABLE for areas where FOOD ESTABLISHMENT operations are conducted;
(2) Closely woven and EASILY CLEANABLE carpet for carpeted areas; and
(3) Nonabsorbent for areas subject to moisture such as FOOD preparation areas, walk-in refrigerators, WAREWASHING areas, toilet rooms, mobile FOOD ESTABLISHMENT SERVICING AREAS, and areas subject to flushing or spray cleaning methods.
CORRECTIVE ACTION: Install nonabsorbent ceiling tiles as required.
