The self-closing mechanism on the toilet room door is broken.
CODE REFERENCE:
(6-202.14) Except where a toilet room is located outside a FOOD ESTABLISHMENT and does not open directly into the FOOD ESTABLISHMENT such as a toilet room that is provided by the management of a shopping mall, a toilet room located on the PREMISES shall be completely enclosed and provided with a tight fitting and self-closing door.
CORRECTIVE ACTION: Repair.

(a) The three-compartment and food preparation sinks in the kitchen are not secured to the walls behind each.
CODE REFERENCE:
(4-402.11) (A) EQUIPMENT that is fixed because it is not EASILY MOVABLE shall be installed so that it is:
(1) Spaced to allow access for cleaning along the sides, behind, and above the EQUIPMENT;
(2) Spaced from adjoining EQUIPMENT, walls, and ceilings a distance of not more than 1 millimeter or one thirty-second inch; or
(3) SEALED to adjoining EQUIPMENT or walls, if the EQUIPMENT is exposed to spillage or seepage.
(B) COUNTER-MOUNTED EQUIPMENT that is not EASILY MOVABLE shall be installed to allow cleaning of the EQUIPMENT and areas underneath and around the EQUIPMENT by being:
(1) SEALED; or (2) Elevated on legs as specified under ¶ 4-402.12(D).
CORRECTIVE ACTION: Secure and seal as required.
(b) There is an escutcheon plate on the wall which is not covered - to right of the three-compartment sink.
CODE REFERENCE:
(6-201.12) (A) Utility service lines and pipes may not be unnecessarily exposed.
(B) Exposed utility service lines and pipes shall be installed so they do not obstruct or prevent cleaning of the floors, walls, or ceilings.
(C) Exposed horizontal utility service lines and pipes may not be installed on the floor.
CORRECTIVE ACTION: Cover as required.

The PIC does not have any QAC test strips.
CODE REFERENCE:
(4-302.14)A test kit or other device that accurately measures the concentration in MG/L of SANITIZING solutions shall be provided.
CORRECTIVE ACTION: Provide these test strips.
