Inspectors Comments: Observed 2 dead cockroaches on the floor in the kitchen. (Also observed 1 dead spider and 2 dead crickets on the floor beneath equipment.) A sign posted on the dorr indicated pest control had conducted routine treatment on 10/2/2020.
A search did not reveal signs of current, live vermin activity.
Eliminate all evidence of former vermin activity (i.e. dead cockroaches) and thoroughly clean and sanitize all affected surfaces.
Construct, equip, maintain and operate the food facility so as to prevent the entrance and harborage of animals, birds and vermin including, but not limited to, rodents and insects.

Inspectors Comments: Observed three live cockroaches and several dead cockroach bodies in multiple cabinets and drawers.
Observed several Combat cockroach eradication devices, which appeared to be for household use.
Discontinue the use of household products and call your licensed pest control operator to address the infestation.
The facility will be closed until the cockroach infestation has been eradicated and the facility has been thoroughly cleaned and sanitized.
Prior to calling your inspector for a re-inspection, ensure that the following things occur:
1.) Treat using approved pest control methods only.
2.) Clean and sanitize all affected areas.
3.) Ensure there are no live cockroaches or dead cockroaches in the facility.
4.) Seal/close all openings, gaps, holes, to prevent the entrance of pests.
