Inspectors Comments: Observed several dead American cockroaches inside multiple floor sinks and behind cooking equipment. No additional evidence of a possible infestation was observed. Inspectors Comments: Observed rodent dropping in cabinet near service station. A search did not reveal other signs of rodent activity. Construct, equip, maintain and operate the food facility so as to prevent the entrance and harborage of animals, birds, and vermin including, but not limited to, rodents and insects. Follow up with your pest control company to ensure activity does not continue.

Inspectors Comments: Observed one large, dead cockroach on the ground inside the pantry/dry goods storage area. A search did not reveal any other signs of activity.
The cockroach was disposed of and the area cleaned and sanitized.
Per discussion with Darren Hughes, Assistant Food Service Director for PUSD, Armed Exterminators were onsite on Sept 25 at 10:00 AM. No activity was found, no dead bodies were observed.
Watch carefully for cockroach activity and contact your licensed pest control operator immediately to eliminate if found.
Inspect packages for cockroaches; one employee had observed a cockroach in a food delivery package approximately 2 months ago.
Facility appeared to be very clean; please continue in your efforts to maintain cleanliness.
Thermometers provided and accurate within 2 degrees
