Inspectors Comments: A puddle was observed adjacent to the back walk-in cooler that appears to be coming from the cooler itself. The origin could not be determined because storage items could not be moved. This leak did not have an odor and appeared to be water, possibly from the condenser. The leak is being temporarily contained/confined. The leak must be repaired immediately. A follow-up visit will be conducted to ensure compliance with this requirement.

Demonstration of knowledge; food handler card All food employees shall have adequate knowledge of and be trained in food safety as it relates to their assigned duties. Food facilities that prepare, handle or serve non-prepackaged food, shall ensure that all food handlers have an approved food handler card. (113947, 113948)

Inspectors Comments: Observed contents of back walk-in cooler at 43 F. Adjust or repair to maintain 41 F or below. Observed contents of walk in cooler at 42 F. Adjust or repair to maintain 41 F or below. Observed pico de gallo in a top insert pan on the service line at 49 F. This product is close to hot equipment. The pico was voluntarily discarded and replaced with new product. A lid was placed on the new product to keep cooler. Item may need to be moved to prevent heating next to hot equipment. Maintain all cold foods below 41 F.

Inspectors Comments: Observed a few large flies by the back exit and several small flies (10-15) around the mop bucket and at the soda dispenser near the check-out area. Use approved methods to eliminate from the facility. Clean to eliminate food residue they are attracted to.

Adequate ventilation and lighting; designated area, usage Exhaust hoods shall be provided to remove toxic gases, heat, grease, vapors and smoke and be approved by the local building department. Canopy-type hoods shall extend 6" beyond all cooking equipment. All areas shall have sufficient ventilation to facilitate proper food storage. Toilet rooms shall be vented to the outside air by a screened openable window, an air shaft, or a light-switch activated exhaust fan, consistent with local building codes. Adequate lighting shall be provided in all areas to facilitate cleaning and inspection. Light fixtures in areas where open food is stored, served, prepared, and where utensils are washed shall be of shatterproof construction or protected with light shields. Thermometers provided and accurate within 2 degrees An accurate easily readable metal probe thermometer suitable for measuring temperature of food shall be available. A thermometer +/- 2°F shall be provided for each hot and cold holding unit of potentially hazardous foods and high temperature warewashing machines. (114157, 114159)

Inspectors Comments: No thermometer observed in back walk-in cooler. Provide accurate thermometers for the interiors of all refrigeration units and place in the warmest part of the cooler.

Inspectors Comments: Observed icing problem in walk-in freezer. Door has been damaged. Repair door/gasket so prevent icing problem from continuing. Observed gaskets in disrepair at back walk-in cooler. Repair within 1 week.

Hands clean and properly washed; gloves used properly Employees are required to wash their hands: before beginning work; before handling food / equipment / utensils; as often as necessary, during food preparation, to remove soil and contamination; when switching from working with raw to ready to eat foods, after touching body parts; after using toilet room; or any time when contamination may occur. Food employees shall not contact exposed, ready-to-eat food with their bare hands and shall use suitable utensils, gloves, or dispensing equipment. Food employees shall keep their fingernails clean. Inspectors Comments: Observed an employee adjust his hat and attempt to put on gloves without washing his hands. The violation was corrected by the employee washing his hands after being instructed to do so.

Inspectors Comments: Measured less than 200 ppm quat sanitizer in the 3-compartment sink during active warewashing. Ensure the sanitizer measures 200 ppm when using quat during active warewashing. The violation was corrected by emptying the water and refilling the compartment with sanitizer.

Inspectors Comments: Sanitizer test strips were not available for use during the time of inspection. Maintain testing equipment and materials in the facility and available for use at all times for the purpose of adequately measuring the types of sanitization methods used in this facility.
