Inspectors Comments: Observed dark debris on interior sides of large upstairs ice machine and interior lid of ice machine. Clean to remove debris and maintain ice machine in clean condition.

Inspectors Comments: Observed cooler behind bar to be repaired with black tape and unable to close properly. Remove tape from cooler. Repair properly and restore smooth, cleanable surfaces. Ensure cooler can shut properly. No potentially hazardous foods stored in this cooler at this time. Maintain all equipment in good working order.

No smoking signs shall be posted in food preparation, food storage, warewashing, and utensil storage areas. Consumers shall be notified that clean tableware is to be used when they return to self-service areas such as salad bars and buffets. Inspectors Comments: OBSERVED SEVERAL DRAIN FLIES IN THE BAR AREA. THOROUGHLY CLEAN THE ENTIRE BAR AREA AND ELIMINATE STANDING WATER, SLIME AND STICKY AREAS. CLEAN THE FLOOR DRAIN, FLOOR SINK AND FLOORS. CLEAN THE BEER TAP STATION AT THE END OF SHIFT. CORRECT WITHIN TWO DAYS.

Inspectors Comments: OBSERVED CONDENSATE LEAKING FROM REFRIGERATOR DOORS AT BAR. REPAIR OR REPLACE GASKETS TO PREVENT CONDENSATE FROM FORMING AND LEAKING FROM UNIT. CORRECT WITHIN ONE WEEK.

Inspectors Comments: OBSERVED BEER TAP WASTE PIPE CURLED IN THE FLOOR SINK. CUT THE WASTE PIPE TO DRAIN PROPERLY. CLEAN THE FLOOR DRAIN AND SINK DRAINS THROUGHOUT THE BAR AREA. OBSERVED GRIME ALONG THE INSIDE OF THE DRAIN PIPES.

Inspectors Comments: Provide a separate area for the storage of the construction materials from food and utensil. Observed an open container of putty and caulking material on the shelf storing clean glasses.

Floors, walls and ceilings: built and maintained The walls / ceilings shall have durable, smooth, nonabsorbent, light-colored, and washable surfaces. All floor surfaces, other than the customer service areas, shall be approved, smooth, durable and made of nonabsorbent material that is easily cleanable. Approved base coving shall be provided in all areas, except customer service areas and where food is stored in original unopened containers. Food facilities shall be fully enclosed. All food facilities shall be kept clean and in good repair. (114143(d), 114266, 114271)

Inspectors Comments: -Observed foam wall fillers covering up the holes and crevices on the wall in the bar area. Ensure smooth surfaces are provided for adequate cleaning.

Inspectors Comments: Clean and maintain the non-food contact surfaces. -Observed heavy dust accumulation on the glasses, and pitchers on top of the bar counter. Operator was instructed to wash and sanitize them. Corrected on site. -Observed heavy dust accumulation on the bar counter and shelves. -Observed dust accumulation on the interior surfaces of the reach-in-cooler, particularly behind the condenser.

Permit reinstated An inspection was conducted on this date at the request of the owner to verify compliance with the reason for closure. The inspection resulted in permit reinstatement.

Inspectors Comments: Today's visit was at the request of the facility manager after a 5/23/16 closure for cockroaches. Observed all the major violations to be corrected at the time of the inspection. Health permit is reinstated on this date.

Equipment, utensils and linens: storage and use All clean and soiled linen shall be properly stored; non-food items shall be stored and displayed separate from food and food-contact surfaces. Utensils and equipment shall be handled and stored so as to be protected from contamination. (114074, 114178, 114172, 114081, 114185, 114119)

Inspectors Comments: -Observed clean bar utensils stored under the dusty panel inside the reach-in-cooler. Properly store all the utensils in a clean and approved manner.

Inspectors Comments: Observed rusted metals racks were being spray painted. Provide documentation of the spray paint stating it's food grade and safe to use inside the refrigeration unit or replace them with approved metal racks.

Inspectors Comments: Eliminate cockroaches from the facility by a safe and legal method at once. -Observed live and dead adult and nymph cockroaches on the floor and inside the cabinet at the top bar. Operator provided recent pest control receipt. The following actions must be completed to properly eliminate the cockroaches from the facility: 1. Construct, equip, maintain and operate the food facility so as to prevent the entrance and harborage of the cockroaches. All gaps and holes in the interior and exterior of the building must be identified and repaired. 2. Thoroughly clean and sanitize all affected surfaces in the facility. All areas must be cleaned with soap and sanitized with an approved sanitizer.

Inspectors Comments: Observed metal shelves inside the refrigeration units heavily rusted. Repair pr replace the shelves.

Inspectors Comments: Clean and maintain the non-food contact surfaces. -Observed a live cockroach inside the wood cabinet. Clean and sanitize the affected surfaces.

Inspectors Comments: Eliminate the leak at the water lines. Observed water leaking from the water lines below the bar counter.

Inspectors Comments: Properly cover all the food items inside the refrigeration units to prevent overhead contamination. Observed uncovered juice inside the refrigerator.

Premises; personal/cleaning items; vermin-proofing The premises of each food facility shall be kept clean and free of litter and rubbish; all clean and soiled linen shall be properly stored; non-food items shall be stored and displayed separate from food and food-contact surfaces; the facility shall be kept vermin proof. Inspectors Comments: Construct, equip, maintain and operate the food facility so as to prevent the entrance and harborage of animals, birds and vermin including, but not limited to, rodents and insects. Seal all gaps that are greater than a 1/4 inch. -Observed loose FRP wall behind the wooden cabinet. -Observed holes on the bottom wall behind the wood cabinet. All gaps and holes in the interior and exterior of the building must be identified and repaired.

Imminent Health Hazard - Closure An enforcement officer may temporarily suspend the permit and order the food facility immediately closed due to an imminent health hazard. (114409)
