Inspectors Comments: OBSERVED A FAN ON THE FLOOR OF THE BEVERAGE PREPARATION AREA. DISCONTINUE USING FANS IN AREAS WHERE OPEN FOOD IS STORED (E.G., REACH IN FREEZER UNIT). VENTILATION SYSTEM SHALL BE DESIGNED AND INSTALLED IN A MANNER THAT DOES NOT CAUSE CONTAMINATION OF FOOD, FOOD CONTACT SURFACES OR UTENSILS, OR CREATE AIR CURRENTS THAT CAUSE DIFFICULTY IN MAINTAINING THE REQUIRED TEMPERATURES OF POTENTIALLY HAZARDOUS FOODS (E.G., PLAIN YOGURT). Permit reinstated
An inspection was conducted on this date at the request of the owner to verify compliance with the reason for closure. The inspection resulted in permit reinstatement.
Inspectors Comments: During a previous inspection the facility was closed due to a sewage back up coming from the floor sink under the 3-comp sink.
Upon todays inspection, all violations were corrected. Employee stated all contaminated areas were cleaned and sanitized. Observed all floor sinks properly draining, and restrooms properly working.
Observed plumbing invoice dated for 4/8/16.

Food contact surfaces: clean and sanitized
All food contact surfaces of utensils and equipment shall be clean and sanitized. (113984(e), 114097, 114099)
Inspectors Comments: Properly clean, and sanitize all utensils. Observed ware-wash sink not being able to drain due to the clogged floor sink.
Sewage and wastewater properly disposed
All liquid waste must drain to an approved fully functioning sewage disposal system. (114197)
Inspectors Comments: Repair the clogged floor drain to properly dispose the waste water from the ware wash sink.
-Observed waste water over flowing from the floor sink due to the clogged drain at the ware wash sink.
Provide operational toilet to the facility at once.
-Observed only available restroom in the facility was out of order at the time of inspection. According to the operator, it has been inoperable for a few months and all the employees have been using neighbor's restroom instead.
Approved toilets shall be available and accessible during all hours of operations.

-Observed damaged/chipped floor tiles in the front food prep area. All floors must be smooth, easily cleanable and non porous.
Plumbing; proper backflow devices
The potable water supply shall be protected with a backflow or back siphonage protection device, as required by applicable plumbing codes. All plumbing and plumbing fixtures shall be installed in compliance with local plumbing ordinances, shall be maintained so as to prevent any contamination, and shall be kept clean, fully operative, and in good repair. Any hose used for conveying potable water shall be of approved materials, labeled, properly stored, and used for no other purpose. Inspectors Comments: Provide an air gap between the drain line and the floor sink. Observed a drain line inside the floor sink without an air gap at the food prep sink.

Inspectors Comments: An office hearing is scheduled for 04/11/16 at 8:00 am to discuss the closure. Please bring the following to the hearing:

-Invoice for plumbing repair

Imminent Health Hazard - Closure
An enforcement officer may temporarily suspend the permit and order the food facility immediately closed due to an imminent health hazard. (114409)

Inspectors Comments: Maintain an adequate supply of hot potable water (minimum 120 F) under pressure at the 3 compartment sink. Hot water measured 110 F at the 3 compartment sink during the inspection.
Garbage and refuse properly disposed; facilities maintained
All food waste and rubbish shall be kept in leak proof and rodent proof containers. Containers shall be covered at all times. All waste must be removed and disposed of as frequently as necessary to prevent a nuisance. The exterior premises of each food facility shall be kept clean and free of litter and rubbish. Thermometers provided and accurate within 2 degrees
An accurate easily readable metal probe thermometer suitable for measuring temperature of food shall be available. A thermometer +/- 2°F shall be provided for each hot and cold holding unit of potentially hazardous foods and high temperature warewashing machines. (114157, 114159)
