Inspection Information

Inspections are based on regulations that are designed to eliminate risk factors for food borne disease. Each violation of these regulations is listed under the inspection.
Violation Types
Priority (P): The highest risk violations which are directly related to an increased risk of foodborne illness. Examples include improper handwashing, unsafe food temperatures.
Priority foundation (Pf): Violations that can create a high risk (Priority) violation. Examples include lack of soap at a handsink, no calibrated thermometer, no test strips to check sanitizer levels.
Core (C): Lower risk violations. Examples include general maintenance of facility (floors, walls, ceilings).
 
Correction Requirements: (P) and (Pf) violations must be corrected immediately when possible. Follow-up inspections occur to ensure all of these violations have been resolved if not corrected during the routine inspection. (C) violation corrections are verified at the next routine inspection.

11/29/2023 - ROUTINE INSPECTION
  3-304.12 - CORE
 Violation Comments:The ice scoop was stored with its handle in the ice. All utensils used for dispensing food and ice must be stored with the handle above the product to prevent accidental contamination. Corrected on site: The person in charge moved the ice scoop so it was stored with its handle out of the product.
  4-602.11 - CORE
 Violation Comments:The white ice shield in the ice bin had a build up of pink mold on it. All equipment that touches non-potentially hazardous food must be cleaned often enough to keep it clean to limit accidental contamination of product. To correct: Clean the ice bin shield more frequently.

  6-501.14 - CORE
 Violation Comments:The ventilation filters above the fryers and grill line had a large amount of grease buildup on them. Ventilation filters must be kept clean and free of buildup so they do not become a source of contamination. To correct: Clean the filters. Ensure to increase cleaning frequency.
  5-205.15 - PRIORITY
 Violation Comments:The drain lines for the ice bin and the main prep sink are hanging below the flood rim of the receiving drain. Plumbing must be maintained in good repair and according to law. An air gap of at least one inch on drain lines for the rinse and sanitize sinks must be provided to prevent backflow of sewage into a food prep area. To correct: Repair the drain lines and ensure there is a one inch gap between the end of the drain line and the received floor drain. Send a photo as proof of correction to hhoeksema@miottawa.org within 10 days.
  5-501.114 - CORE
 Violation Comments:The large outdoor dumpster is missing a drain plug. When looking directly at the dumpsters, it is the one on the right. Dumpsters must have a drain plug installed to prevent attraction of pests. To correct: Contact your servicer and have a drain plug installed in the dumpster indicated above.
